Model and Interview Release Form
By filling this form, I hereby grant to ARIANE AUDET (“Interviewer” and/or “Photographer”) full rights of my images and interview to Faces of Postpartum Inc.
I consent to the recording of my statements and grant to ARIANE AUDET and Interviewer’s assigns, licensees, and successors the right to copy, reproduce, and use all or a portion of the statements (the “Interview”) and images (“Photographs”) for incorporation in the following work FACES OF POSTPARTUM INC. (the “Work”).
Unless otherwise stated in the “Additional Comments” section below, I permit the use of all or a portion of the Interview and Photographs in the Work in all forms and media including but not limited to portfolio, publicity, illustration, advertising, and blogging content throughout the world and in perpetuity. I waive the right to inspect or approve use of my Interview and Photographs as incorporated in the Work.
If model(s) is under 18 year of age, I state that I am the parent or legal guardian of the minor named below. I have the legal right to consent to and do consent to the terms and conditions of this release.
